This is possibly the glitch that Gmod 13 introduced, it crashes the server when a ragdoll glitches out a ton, players can do this purposely or accidentally. All of my Garry's mod servers had the same issue, it is a simple line of code to add, which you can find on Facepunch or Google search by looking up "Garry's Mod TTT Crash Glitch". It is a ragdoll glitch that happens in Gmod 13 and hopefully Garry will fix this soon.
