Focus on the sv_parallel_sendsnapshot command. On high-traffic servers this can absolutely tank your server fps. On my 60 slot CS:S server this set to 1 provides fluid and smooth gameplay at spawns with 50+ people. Set to 0 causes us to hover between 25-35. I have had first hand experience with vds ...

I've seen sql cause noticeable cpu usage spikes on a quad core machine, I would recommend putting sql on a different machine, if possible.

do you have something rcon into the server periodically?
Make sure it is the last line of the server.cfg, and check the gamemodes configs for it as well.

Do you have sdkhooks installed?
try doing
sm plugins list
in the server console(through nfos control panel) and post the result, it will sometimes say if a plugin failed to load

It probably loses connection with valves matchmaking system. I've heard of this happening in TF2 as well, set NFO to reboot your server every morning when its empty via the managed panel(if you have that)

Since the servers run on a single core(most of them), if a server is full/highly loaded it will take up 100% of 1 core, instead of spreading it out. That might explain what you are seeing.
