I haven't had the chance to do much testing with SourceTV since the OB CS:S update, but in my quick test just now, it seemed to work. I used these basic commands in the server.cfg, then restarted the server to turn it on:

tv_enable 1
tv_maxclients 1
tv_password "password"
tv_transmitall 1
tv_name "Test SrcTV"
tv_delaymapchange 0
tv_delay 10

Are you seeing the SourceTV in the server, through HLSW or another external querying system?

This is on a plain vanilla server without plugins? And you've done a -verify_all with hldsupdatetool on it?

If so, I'm a bit stumped, since this isn't something I've been able to duplicate here, and the new version is still only a couple of days old.

You have tv_name in there twice and also tv_title. Those may be conflicting with each other, try removing all but one of the tv_name.

I change that still cant get it to work. but the funny thing is it will enable it self out of the blue. It was working but not right away. Like right now tv is enable on both of the scrim servers we have. It stats in console that tv is enable when i stop or restart the server, it add an additional slot to make up for the tv.
so i dunno lol

In the past, a server restart would normally kick the TV. It would then take a map change to get the TV running after a server restart.

The next time you see this come up, try doing a map change to see if that helps. It may also be some odd quirk where it doesn't start running until both teams have players, or some such.
