Dudes.. I don't have this in maps.ini, standardmaps.ini, or mapcycle. Nor do I see in our maps dir.. But somehow our server goes to this map.. Any ideas?

That's a very odd problem. Undertow is a HL:DM map that resides in the Valve directory. Are you certain that your mapcycle is valid, and that no one is changing to that map?

Under what exact circumstances do you end up on that map?
