Server Massively Lagging?

Hi, so my friends and I were trying to figure out why our server is lagging so badly...

This is our CPU/memory stats:
CPU usage: 13.28%
Free physical memory: 12406mb

We have the Intel Xeon E3-1270 with 4 processors...

And it's a managed VDS..

Oh, and it has ~60 people on it.


If anyone could provide any assistance, that'd be great...
All our addons are popular addons, so I don't believe the error is within that area.

When does the lag start occurring? Is it constant, or only after X amount of people join? Are you seeing any choke/loss in net_graph 4? Also, you'll want to check the server usage tab in your control panel to see what the game server is actual using, the server health tab shows the entire VDS.
