Just as a test would you mind disabling SM/MM to see if that helps at all?

After I wiped the server, I let it run for a day without sm+mm, and nobody was crashing

That sounds like it narrowed down the cause to something added related. The next step would be to run Metamod on there without Sourcemod, and then keep progressing from there.

I'm seeing the same thing AND I'm also seeing frequent server crashing, particularly after a kick or ban by an Admin.

The thing I can't understand is how there's nobody else having my issue... I didn't do anything weird when pasting in metamod and sourcemod

I'm looking through the sourcemod changelogs now... no clue on what could cause this from the older versions... There is also one high ranked server using the latest snapshot like me though :/

Okay... It may actually just be a sourcemod problem with the latest csgo patch. Other people are now coming up in google stating they are crashing on other servers!

So basically I just wasted a day or two and broke my server for no reason -_-

That is the unfortunate result of mixing addons and game updates. CS:GO being a bit newer seems to have slightly more trouble in that regard, but it is certainly not alone. :(
