Custom VS Hardcore

I had my server displaying Hardcore, now it has changed to Custom and I don't recall making any changes.

Did you check hardcore refresh preset every 5 minutes? If not do so and restart server.

If you check the "Set hardcore" box on the page, it should change settings that would be breaking that mode for you.
