how to ban people from private server?

Assuming this is a Half-Life based server..
Ban:
rcon banid # STEAM_0:X:XXXXX kick
# = number of minutes to ban, 0 for permanent

Unban:
rcon removeid STEAM_0:X:XXXXX
rcon writeid
