Lately I got malware installed on my webserver I am renting here on NFO. It was a bit of a pain to figure it all out at first, but here is what I learned that made it easy to remove.
First, I got Google Webmaster Tools (it helped tell me the exact line of code to look for so I can remove it by hand). Then I got Dreamweaver (it helped me be able to download my website and then use replace all to find and automatically remove the Malware). Lastly, I used Google Webmaster Tools to tell Google to re-review my website so Google stops warning people of my website being full of Malware.
After I did those two simple things, I was clean of all Malware and it seems to have been gone for now.
How did I get it? I am not sure, but it might have been cause of phpMyAdmin, so I got rid of that and seems to be all good now.

Also important is to keep all of your software up to date, as much as you can. Historically, some software packages have had a lot of vulnerabilities and are commonly exploited -- phpMyAdmin is one, as you mentioned, and Wordpress and vBulletin have also seen multiple critical bugs.
