Getting My Files Removed?

I have been helping a gaming clan out, which has their website hosted here at nfoservers. I uploaded my own personally created php code files and images to their server, with the conditions of retaining the copyright and revocation of usage privileges myself; the headers of all uploaded php files state this in the comments. If the owner of that website would refuse to remove my php code files and images, who would I contact about getting these files removed from that hosted server?

You could file a DMCA takedown notice about this, most likely. We are not attorneys and can't directly advise you on that matter, though -- I don't know if the DMCA specifically applies to your case.

Where would I send the DMCA; I don't see any forms or anything for that on the nfoservers.com website.
