i have done everything right check website above but its not connecting to the website at all

The database im trying to connect is "lgk_amxbans"
I added the password correctly, nfo staff's checked it everything was right.

They said this "This does look correct, but what is attempting to connect to MySQL with the username "lgkclan" in the top half of your log?" for the reply after i sent them my SQL info.
And after i asked for help they said this "I don't know what you have that might be connecting with a different username, but I suspect it's conflicting and likely causing your problem.
