We recommend it for all servers. The extra ticks on the server increase its responsiveness considerably, which leads to faster and more predictable hit registration, among other things.
Does this mean that your hitbox wont be right/off balance?

The ping accelerator increases the accuracy, it does not "change" the hitboxes but allows for higher accuracy when shooting at them.
