GarrysMod Engine Error broke sounds, and possibly other NFO

Hello everyone I come here in need of support, not about how to use ulx or something simple like that but with something I really need help on and even though I have 3 years in working with source I have no idea what the issue is here. So I'll go ahead an explain the entire thing in great detail:

So we were playing on a map called Crummy Cradle where there is a collapse-able bridge on it. A player carrying a dead (ragdoll) body was playing around with this bride because if you bash the dead body on the bridge goes flying. So he does this, the bridge flies up and is about to land on him. To try to save himself he raises the dead body over his head to stop the bridge but the bridge landed on him and crushed him, killing him. What I've read and gathered from support was that if a player dies (becomes a ragdoll) inside of another dead ragdolled body it will crash the server due to an engine error. This can also happen when a body is moving too fast or hits the ground after moving too fast. I found people complaining about this here too. It seems as if it can be prevented with "cl_ragdollcollide 0" but that's only preventing it. The next paragraph explains what happened after the server restart.
So what this caused that is really bugging me is as follows. The shotgun (and everything else in the list below) can only be heard by the player shooting them. For example if I shot a shotgun I could hear it but a player next to me couldn't. They would only hear the sound of the bullets hitting the wall. Not the shotgun shot itself. Here is a list of all known broken sounds:
1. M16 Gunshot Sounds
2. Shotgun Gunshot Sounds
3. Glock Gunshot Sounds
4. Pistol Gunshot Sounds
5. Mac10 Gunshot Sounds
6. Deagle Gunshot Sounds
7. Grenade bouncing sounds but they cant be heard by anyone
Oddly enough the Rifle's sounds worked. I thought maybe either CS:S sounds or HL:2 sounds broke but it seems like I can hear C4 (CS:S) beeping sounds and the poltergeist shot sound (HL2). Deleting my addons, as expected didn't fix it and neither did a full server repair. After reading the thread that I also noticed that it affected my testing server, currently named lkstf2 to any staff seeing this. My test server which never crashed from the error had this issue which lead me to beleive maybe it was my gmod. I tested this out with a friend on a listen server I quickly made but the sounds were working just fine there so it must be an issue with the server. I am currently completely reinstalling a fresh copy of my gmod and am going to schedule a wipe on my server to hopefully fix it but does anyone know a solution to this? I also reinstalled Trouble in Terrorist Town to make sure it wasn't that but still, no dice. Thanks to any help in advance. Ill keep posting some stuff here saying what did and did not work.

I'm just saying it's really odd how it happened on a server where the crash didn't even occur though. If anyone would like to test it out just message me and you can join my server to see yourself.

I have ragdolls crash my server too. I don't think cl_ragdollcollide has fixed it for many people as I know it still crashes for me. I remember having an issue like this one time, but I think a server restart fixed the issue. I am not sure why you would be continually experiencing this problem. Another time, my Gmod server just started repeatedly crashing over and over again, giving Core Dumps. The only way I was able to fix it was with a restore from a previous backup.

Fresh gmod didn't fix it. Time to move on to a full server wipe xP 3rd one this week! Ughhh

So a complete server and client reset did not fix it. I'm really just throwing in the towel on this one. I'm gonna be working on a work around using Battlefield 3 sounds and m9k weapons. If people need it ask me and I'll send you it when it's done but NFO should really be looking for a fix on this.

Out of curiosity, if you put cl_ragdollcollide 0 back to 1 does it fixes it? It seemed this occurred after that.

Just as a test, does running in Sandbox with a stock map solve the issues?

Kraze, I believe that the only fix that was found was a Lua script to remove rag dolls that start to glitch out. The thread is found here http://facepunch.com/showthread.php?t=1269050&page=5

4 completely stock different users on a completely stock server (neither the player or client had any custom con latent what's so ever) still had the issue on sandbox. The script Josh put only fixes the crashing, not the sound. The sound is a major feature for TTT since you need to know when, how, and where people die to okay the game correctly. If traitors or silently killing people all the time then it is no fun.
